I'm pretty sure production designer Tommy Lee Wallace actually went out and bought the mask. I think he also went on to direct a couple of the sequels.

It was Tommy Lee Wallace I was looking for. Here, I think this will help out: Don Post was offered production points in the film in exchange for a custom-made mask. Declining the offer, he suggested instead to refurbish existing masks for the movie. Wallace was then sent out to local costume shops for a cheap mask, and came back with two: Shatner's and the rejected one. Good one Heineken. I could see where Post would be true, but the clue I left was that Carpenter personally thanks him. He does this in Bravo's 100 Scariest Movie Moments, in the Halloween segment. That was the clue I was going to give if you hadn't guessed it by now: that he was a director of one of the sequels. Give it to him, Q.

Yeah, he said in an interview that if he ever took the crew up on their offer, the exposure and business from other companies, plus the royalties he would've earned would've been close to $100,000. Talk about fucking up badly.

It was not actually filmed there, it's a fictional town. And again, for the record, the movie is mostly set there, but the entire movie does not take place there. Just a big chunk of it. There are a couple of other locations in the movie.
Oh, and no, not BRIDGES OF MADISON COUNTY.
